Identifying and managing the needs of at-risk or asymptomatic people is an important aspect of the continuum of care model which includes the early recognition of symptoms and prompt referral to allied health professionals, specialists and education programs.
People with complex or multi-morbid conditions will be rehabilitated as far as possible or to reduce the likelihood of exacerbations or relapses for existing/chronic conditions.

Karen is a passionate nurse advocate and mentor with extensive experience in Primary Health both as a Practice Nurse and Primary Health Care Nurse Coordinator and Educator.
Karen has undertaken extensive professional development studies including Immunisation, Travel Medicine, Triage, Chronic Disease Management, Care Planning, Diabetes Self Management , Long Acting Injection (LAI) Intramuscular Injection Training for RNs (Train the Trainer, Mental Health).
Hilda Groenewald is also part of our Chronic Disease team of Nurses.
