Our aim is to improve patient outcomes by facilitating multidisciplinary care for people with chronic and complex conditions.

The primary objectives are to:

  • Prevent and/or delay the onset of chronic disease for individuals and population groups
  • Reduce the progression and complications of chronic disease through early detection and minimising risk factors
  • Maximise the wellbeing and quality of life of individuals living with chronic disease and their families and carers
  • Reduce avoidable hospital admissions and health care procedures through Care Coordination
  • Implement best practice in the prevention, detection and management of chronic disease
  • Enhance the capacity of the health workforce to meet population demand for chronic disease prevention and care into the future

Identifying and managing the needs of at-risk or asymptomatic people is an important aspect of the continuum of care model which includes the early recognition of symptoms and prompt referral to allied health professionals, specialists and education programs.

People with complex or multi-morbid conditions will be rehabilitated as far as possible or to reduce the likelihood of exacerbations or relapses for existing/chronic conditions.
